New Patient Registration
If you live in our practice area and would like to register, you can do so by registering online by using the following link https://gp-registration.nhs.uk/L83106
Alternatively you can complete a registration form which can be downloaded here to print at home or collected form our reception desk New Patient Registration Form .
Please complete a New Patient registration form for every person wishing to register – including children.
If registering via the paper form, once completed, please return the form to the surgery along with proof of identification.
Registration cannot be accepted unless ALL the questions have been answered and we ask for proof of identification has been presented to the surgery.
The Receptionist will confirm your usual Doctor with you. You will be registering with the practice rather than an individual GP.
PLEASE NOTE THAT COMPLETING THE NEW PATIENT QUESTIONNAIRE DOES NOT AUTOMATICALLY MEAN THAT YOU WILL BE REGISTERED WITH THE SURGERY.
Once registered you are entitled to a new patient consultation – this is not compulsory but if you are taking regular medication, it is important to arrange an appointment before your current medication runs out so that your repeat prescription can be organised in time.
